Signing up - All students who sign up go on the wait list until medical forms are cleared. Although the website has a default message saying the team is full, that is not the reason for going to the wait list. Please read info below on medical clearance for more details.
The MJSAO board is actively attempting to implement new fundraising ideas in the hopes of being able to charge reduced prices for the 2016-17 school year. We will ask for payment at the parent meeting which will be held after an early season practice. Sports offered at MJS:
Fall: Boys Soccer (grades 7 and 8), Girls Soccer (grades 6-8), Field Hockey (grades 6-8), Cross Country (grades 6-8).
Registration period - May 26 - Aug. 1
Season - start of school through late October
Early Winter: Boys Basketball (grades 6, 7 and 8), Girls Basketball (grades 6, 7 and 8).
Registration period - early - mid October
Season - approx. first week in Nov. - first week in feb.
Late Winter: Boys Volleyball (grades 6, 7 and 8), Girls Volleyball (grades 7 and 8).
Registration period - mid-Dec. - early January
Season - approx. Feb. 1 through mid- March
Spring: Boys Lacrosse (grades 7 and 8), Girls Lacrosse (grades 6, 7 and 8).

MJSAO is offering an opportunity to all 6th, 7th and 8th grade students to be evaluated for a roster position on the MJSAO basketball teams. The teams will meet a total of three days a week for a combination of practices and games. They will compete in the GMCJSCA middle school league.
Try Out Information: Schedules can be found on the mjsdodgers.com website. Please arrive at least 15 minutes early to sign in and warm up. All notifications of cuts will be done via email and by assigned try out numbers. Students should not wear clothing with their name or any travel/club/team name. Team selection is based solely on talent displayed at the tryout. Grade level is not a factor. Parents and/or Spectators are not permitted at the try out.
Try Outs: Exact dates and times will be announced shortly. Earliest start date will be November 14.

Schedule: Weekly schedules vary and will be posted at www.mjsdodgers.com. MJSAO does not provide transportation and parents are responsible for transporting their children to practices/games.
Location: Most practices will be held at MJS, while the auxiliary gym at the high school will occasionally be used when available to avoid having to split the court at the junior school.
Cost: MJSAO is a pay to participate athletic organization. Costs are related to the team’s budget including uniforms, coach salary, official fees and league fees. Approximate cost is $250-275. Financial options/assistance is available for those that qualify. Application forms can be found on the MJSAO web site, will be confidential and must be submitted by the last try out date. Payment of fees shall occur between the pre-season parent meeting after the team is selected and the first game. There will be no refunding of any participation fees.

Registration: To register for the team, please complete the online registration form and submit the medical forms between October 4 and October 24. Forms can be found at www.mjsdodgers.com under Forms. All medical registration materials must be in an envelope marked “MJSAO” and deposited in the MJSAO mailbox in the school’s office.
Athletic Pre-Participation Physical Form (Part A&B) Health History Update (If you already have a current Physical Form on file with MJSAO and it will not expire before Nov. 14, you may simply submit this form instead)
Please note: NJ State requires that any student intending to participate in an extracurricular activity have submitted a standard athletic physical form completed by the parent and examining physician. The exam had to have taken place within 365 days of the first practice/tryout. A nurse and school-hired physician will then review the form. Faxed or photocopied forms are NOT acceptable. The examining physician must sign the form with an original signature—no stamped signatures will be accepted. Forms will be returned for incompleteness.
To facilitate tracking and communication of the medical clearance process, allnew registrations will be placed on a "waiting list" until the athlete has been cleared by the nurse and school physician to participate. The website automatically notifies the person signing up that the program is full - that is just a default setting. The program is not full; if that message has been received, the student is registered and they are on the wait list until medical forms are approved.
When MJSAO has been informed that a student has been cleared, they will be removed from the waiting list and his/her parent(s) will be notified via e-mail of the change in status.
You can also at any time check your child's status by clicking on Edit My Account on the main menu and logging in with your e-mail address and password. If the program you registered for indicates that your child remains on the wait list, he/she has not yet been cleared to participate.
Please be sure to complete the required medical forms – either the Athletic Pre-Participation form (this should be filled out by your physician) or the Health History update form. These are available at http://www.madisonpublicschools.org/Page/3982. For each sport in which a child registers, new medical forms must be submitted. Depending on the date of the child's last physical, either the short update form or the full physical form needs to be turned in.
The forms are reviewed by the school nurse and then sent to the school physician who approves a child to participate. This process takes several days. Please see that medical forms are submitted by the registration deadline to make certain that your child will be cleared in time for the start of the season.
